Job Summary

Under the general supervision of the Clinical Supervisor of the Roving Team, the Social Work Associate (SWA) works with high risk mentally ill and/or substance abusing clients living in permanent supportive housing to prevent homelessness and link them to appropriate outpatient services. Through community outreach, the SWA assists with providing individual intensive clinical case management, counseling, crisis intervention, 5150 assessment, linkage to substance abuse treatment, psycho education; intake, assessment, and individualized service planning to their caseload. The SWA coordinates closely with hotel support services and property management staff, as well as Roving Team members and other outside providers, to develop successful treatment strategies; treatment advocacy and coordination with other agencies and natural supports. The SWA tracks client contacts, treatment plans, assessments, evictions and makes appropriate referrals. The SWA maintains client records according to California, City and County requirements; and participate in an interdisciplinary treatment team. Candidate attends weekly staff meetings and participate in all required training sessions. Plus, other duties as assigned.

Department Description

Citywide Case Management Programs is a Division of the UCSF/ZSFG Department of Psychiatry, located at 982 Mission St. Its mission is to provide behavioral health intensive case management services to adults who suffer from serious mental illness, co-occurring substance abuse disorder and chronic medical conditions, experience significant systemic determinant of health, including justice-involvement or other forms of institutionalization, poverty and housing instability.

Required Qualifications

* Possession of a bachelor's degree in Social Work from a program approved by the Council on Social Work Education, and one year of experience; or an equivalent combination of education and experience; and knowledges and abilities essential to the successful performance of the duties assigned to the position.
* One year working with severely mentally ill adults, the homeless and dual diagnosed populations
* The ability to do outreach, including to SRO hotels
* One year Case Management experience
* Ability to work cooperatively and communicate effectively with others. Ability to work in a high stress, high volume area and work well under pressure and with deadlines. Ability to follow oral and written instructions accurately, and be detailed oriented. Able to efficiently prepare written summaries of activities, set priorities and complete reports within required timeframes. Knowledge of medical and/or psychiatric terminology

Preferred Qualifications

* Demonstrated ability to work within multidisciplinary team to support clients
* Experience with Avatar Billing System
* Prior related experience in the homelessness response system of SF
* Demonstrated understanding of Harm Reduction, The Recovery Model and Housing First Model
