The ZSFG Age-Friendly Emergency Department (AFED) Initiative is a philanthropic foundation funded grant to implement age and dementia-friendly initiatives into the daily operations and practice of emergency medicine and nursing for patients cared for at ZSFG. Over the course of three years, the ZSFG Emergency Department will become a level one certified AFED offering age-friendly medical, nursing, and support services to older adult patients and those suffering with dementia. The Patient Navigator acts as a communication liaison between treatment providers and maintains ongoing relationships with/offers support to enrolled patients. The Patient Navigator will assist the analyst with tracking patient data, as well as working with the team's Project Manager to complete documents needed for reporting to funders and accrediting bodies.
