This position will be responsible for oversight of the RCFE network development and maintenance for IOA's Community Living Services in all active regions. It will also develop relationships with new health plans for CalAIM expansion.SPECIFIC RESPONSIBILITIES:
  • Leads a team of Outreach Coordinators and Admin staff across the entire state in order to develop a network of RCFE
  • Works directly with IOA Procurement to ensure that all contracts are managed with consistency, completeness, and timeliness
  • Develops tracking tools and internal monitoring processes to ensure compliance with all relevant external regulatory entities
  • Acts as primary liaison between IOA and community partners such as August Health as we seek out partnerships to strengthen the IOA RCFE Network.
  • Builds and fosters relationships with health plans and community networks to explore partnership opportunities beyond the RCFE Network
  • Develops and oversees division vendor network and maintenance
  • Develop and implement standardized documentation auditing practices
  • Responsible for preparing and presenting operational reports to internal and external parties such as health plans, state agencies, community partners, etc.
  • Responsible for developing processes to optimize invoice management and invoice review
  • Development and maintenance of RCFE networks in all active regions
  • Creation and management of contract tracking tools
  • Coordinates with external funders on network development progress
  • Supports CalAIM expansion and infrastructure needs
  • Supports waiver agency applications, changes, and expansions including ALW, CCT, and others as needed
  • Supports ALW applications and expansion efforts including engaging DHCS to increase successful application completion
  • Engage Ombudsmans to ensure compliance, safety, and best quality care is maintained throughout the geography
  • Partners with CalAIM Leadership (Directors, Managers, and Supervisors) to ensure program needs are being met by the vendor/RCFE network
  • All other reasonably related jobs as assigned

EDUCATION:
  • Master's degree or higher in Administration, Public Health, Social Work, Rehabilitation, Gerontology, Social Work or related fields from an accredited college or university preferred

BACKGROUND AND EXPERIENCE:
  • 4 years of experience working with the frail elderly and/or adults with disabilities
  • Experience working with community case management programs preferred
  • Experience with contract creation and management
  • Familiar with contract language creation and adherence
  • Experience with oversight of program fiscal operations preferred
  • Intermediate level skill in using Microsoft Excel required (e.g., able to develop multi-sheet workbooks, cell formulas, conditional formatting, filter/sort, what-if analysis, etc.)
  • Intermediate level skill in using Microsoft PowerPoint preferred (e.g., create new presentations, embed charts, slide transitions etc.)
  • Experience working in non-profit organizations preferred
  • Detail-oriented, ability to manage multiple tasks at once, and desire to be extremely organized
  • Individual must have strong communication skills; must possess the ability to communicate effectively in writing and in person with both fiscal and clinical staff
  • Interest and ability to work in a multilingual, multicultural environment

ADDITIONAL REQUIREMENTS:
  • Ability to comprehend complex written material and verbal instructions Ability to communicate and speak clearly in English

PHYSICAL REQUIREMENTS:
  • Ability to lift up to 20 lbs. Ability to reach, bend and walk Five finger manipulation

COMPENSATION:

  • Range: $59.00 - 71.00 USD Hourly
