Job Summary
The Nurse Practitioner will work in Lung Transplant & Interstitial Lung Disease.
One of the following National NP Board Certification is required for this role ANP-C, ANP-BC, FNP-C, FNP-BC, AGACNP-BC, ACNPC.
Lung Disease
The post-transplant Nurse Practitioner and is responsible for the care of the post-lung transplant patient population. Patient care responsibilities include evaluation and management of patients who have received a lung transplant, follow-up of post-transplant clinical testing and immuno-suppressive management and ongoing discharge teaching and clinical assessment of this population. The NP will provide advance practice care under the direction of the transplant faculty. Close collaboration with the medical staff (including referring physicians) and in-patient clinical coordinators is essential to ensure appropriate follow-up care and transition of the care within the clinic. When not in clinic, the NP will be responsible for managing patients' immunosuppression via lab review and patient discussions. The NP is the key point person for patient and may refer the patient to social work or for insurance counseling as necessary. Patients will be monitored and documented through Apex/Epic systems.
Interstitial Lung Disease
Under the direction of the Interstitial Lung Disease (ILD) Medical Director, the Nurse Practitioner (NP) will practice at the UCSF Lung Health Center (LHC)'s and will see patients in the ILD program (across three physical campuses, Parnassus, Mt. Zion and Berkeley Outpatient Practice), with as needed cross-coverage for other LHC APPs. This is a unique opportunity to work closely in a multi-disciplinary team including ILD physicians, social workers, nurses, radiologists, pathologists, and rheumatologists. The NP will work closely with the support of the physicians to evaluate patients in virtual and in-person routine and urgent clinic visits to provide ongoing health assessments,
manage patients with interstitial lung disease and their symptoms, triage acute symptoms (including MyChart and telephone messages), arrange as needed hospital admissions, and provide health maintenance interventions. The NP will also provide assessment and management for patients via telehealth visit, telephone and MyChart, and facilitate care coordination to achieve high-quality care. Management of ILD will involve ordering diagnostic testing such as chest X-rays and CT scans, evaluating oxygen needs with walk tests and nocturnal oximetry, evaluating lung function using pulmonary function testing. NP will learn about ILD diagnosis and management, and how to interpret pulmonary function test and CT scans. The NP will participate in the weekly multi-disciplinary conference dedicated to discussing the evaluation and management of our patients. The NP will also participate in the ILD support group and clinic improvement efforts related to quality of care and patient experience of patients. The NP will review and communicate lab results and diagnostic testing with patients and collaborating clinicians. The NP will arrange and order treatment including medications, durable medical equipment (e.g.
supplemental oxygen therapy and CPAP) based on clinical assessments and diagnostic testing.
The final salary and offer components are subject to additional approvals based on UC policy.

Please note: The compensation ranges listed online for roles not covered by a bargaining unit agreement are very wide, however a job offer will typically fall in the range of 80% - 120% of the established mid-point. An offer will take into consideration the experience of the final candidate AND the current salary level of individuals working at UCSF in a similar role.
For roles covered by a bargaining unit agreement, there will be specific rules about where a new hire would be placed on the range.
